About Dr. Michael B. Mefford

I am a board-certified psychiatrist with more than two decades of experience in adult psychiatry, telepsychiatry, academic medicine, and private practice. Over the course of my career, I have worked in community mental health settings, large multidisciplinary organizations, and solo practice environments. I now focus primarily on the evaluation and treatment of ADHD in high-functioning adults.

Many of the professionals I work with are physicians, attorneys, executives, entrepreneurs, and creatives. They are often intelligent, capable, and accomplished — yet privately frustrated by inconsistency in focus, organization, or emotional regulation. My work centers on clarifying these patterns carefully and treating them thoughtfully.

Training and Clinical Background

I completed my psychiatry residency at the University of South Carolina School of Medicine, where I served as Chief Resident. I then completed fellowship training in Child and Adolescent Psychiatry at the University of Texas Health Science Center in San Antonio.

Throughout my career, I have practiced in both inpatient and outpatient settings, supervised trainees, and helped implement telepsychiatry programs early in their development. I adopted telepsychiatry in 2013 to maintain continuity of care for patients during frequent relocations related to my spouse’s Air Force career.

My clinical experience spans child, adolescent, and adult psychiatry. Over time, my practice has naturally evolved toward adult ADHD and related executive function concerns.

Approach to Care

I believe careful diagnosis matters.

Adult ADHD frequently overlaps with anxiety, depression, trauma exposure, sleep disturbance, and chronic stress. Online screening tools are helpful starting points, but they are not diagnostic instruments. My evaluations are extended and structured to assess developmental history, current impairment, and differential diagnosis.

Treatment is individualized and grounded in evidence. Medication decisions are made thoughtfully and monitored carefully. When appropriate, I incorporate practical strategies that support executive function and long-term stability.

My goal is not to change personality. It is to reduce unnecessary friction in capable minds.

Private Practice Model

I operate a direct-pay practice in Mount Pleasant, South Carolina, offering in-person visits as well as secure telepsychiatry throughout South Carolina.

Patients work directly with me for all aspects of evaluation and treatment. There are no care coordinators, rotating providers, or corporate oversight structures. Follow-up visits are scheduled according to clinical need and regulatory requirements, with a focus on efficiency and continuity.

Many of my patients value privacy, discretion, and consistency. This model allows for that.

A Final Note

Over more than twenty years in psychiatry, I have seen how untreated executive dysfunction can quietly shape careers, relationships, and self-perception.
